    Application Eligibility 

    Ministry of Health
    Those who are currently enrolled as a PGY2 in a Canadian
    Family Medicine Residency Program at the time of application.

    Residents must start their PGY3 year within 12 months of
    completing their PGY2 year.

    Application Requirements:

    1. Completed Online Application Form
    2. Letter of Intent
    3. Current CV
    4. Medical Transcripts
    5. Three (3) references*, one which must be from your Residency Program Director

    * Please note that referees will be submitting references online via the new online application system. Referees will receive a link through the system to submit the references. Reference links will not be sent to referees until an application has been submitted and a confirmation has been sent to the applicant. Referees will have 2 weeks from the date of the application closing to submit their references. Based on the program of application, referees will be requested to either complete a template reference form or upload a letter to the online system.

    In past years, many DFCM Enhanced Skills fellows have successfully incorporated a DFCM continuing education (CE) or Masters Degree program alongside their clinical fellowship program. Such an arrangement would require a separate application into an academic fellowship or graduate studies program as well as approval of the Enhanced Skills Clinical Program Director. Application deadline for the 2016-17 MScCH (FCM/HPTE) degree is Fall 2015 for a Summer 2016  and January 2016 for the Fall 2016 start. The application deadline for the MPH (FCM) degree is January  2016 for a September 2016 start.
